This presentation explores the vital role of care management in supporting older adults and their families, with a focus on the realities of elder fragility and the impact of care-giving burnout. It highlights the physical, cognitive, emotional, and social challenges that come with aging, as well as the toll care-giving takes on family members’ health, finances, and well-being. Through case examples and practical strategies, the talk demonstrates how care management provides comprehensive assessment, planning, coordination, and advocacy to reduce crisis, ease caregiver stress, and promote dignity and independence for elders.
