Signed by the Grand Master, this unframed certificate includes the Eagle Recipient's name and date of recognition (date Grand Lodge recognizes award).

- Name of Scout as it should appear on the certificate;
- Date Eagle was earned;
- Date of presentation;
- Location of Presentation;
- Eagle Scout's Troop Number and Town;
- Council Name;
- Council Address (city/town/village);
- Masonic relationship of Eagle recipient.
